'Wo genau tut es weh?' means 'Where exactly does it hurt?' The others ask about the doctor's office, if you have pain, or how long the appointment lasts.

"Ich messe Ihren Blutdruck." means "I will take your blood pressure." The other options mean "I am writing you a prescription," "I am taking a swab," and "I am checking your pulse."
